Meet Dr. Putnam for a comprehensive evaluation of your health. Necessary tests will be ordered at this visit.
After careful review of your history and test results a personalized plan will be formulated.
You will review the plan with Dr. Putnam during a short appointment in the office or remotely.
During this appointment your personalized plan will be fine tuned. Follow up tests may be ordered and your plan will be adjusted accordingly.
During these visits you will work with Dr. Putnam to further improve your symptoms and achieve your health goals.
Once you are on your path a minimum of yearly follow ups are recommended paired with testing if needed.
